•  If a health care professoinal at Fern House Surgery has requested your blood test, then you do not need to collect a form beforehand. This will be printed at your appointment.

• For blood tests at an outpatient / hospital site, you must collect your blood test form from the surgery reception. Please bring an ID with you. 
• Only book appointments for Mid Essex sites (Braintree, Chelmsford and Maldon)
